An innovative hair transplant technique has been exclusively available in Italy for a few months now. Holland is where it was born and was patented by Dutch researcher, Conradus Chosal Gho. The name of this technique is HST which means Hair Stemcell Transplantation. Another name is PL FUT: Partial Longitudinal Follicular Unit Transplantation.

It is commonly known as Hair multiplication or Dr. GHO technique. For several years it has been practiced with great success in the countries of northern Europe, in London in Great Britain, in Maastricht and Amsterdam in Holland and Paris and Cap d’Antibes in France. Over 15000 interventions in recent years constitute an important case study of results.

THE DIFFERENCE WITH TRADITIONAL HAIR TRANSPLANT TECHNIQUES

It is a technique that, like FUE, is applied to individual follicular units, but differs fundamentally from FUE. The latter takes entire follicular units from the donor area and moves them from the harvesting area located in the patient’s neck to the recipient area. With the HST technique, whole follicular units are not taken, but portions of them. To do this, needles are used, which in jargon are called punches. They are much smaller in size than even the smaller ones used in FUE.

Their size is half a millimeter, and, in fact, only a portion of the follicular unit is harvested. By taking portions of follicular units, we know that each of them contains hair stem cells. Stem cells are present both in the harvested portion, intended to be transplanted, and in the non-harvested portion. It is from this knowledge that the concept that led to hair multiplication was derived. In fact, each group of stem cells generates over time a follicular unit equal to the one of origin.

THE MULTIPLICATION OF FOLLICULAR UNITS

From one follicular unit, therefore, two are born, and with this principle it is said that there is a multiplication of follicular units. This concept of almost cloning of the hair/follicular units makes it possible to find a solution in the unfortunately very frequent and insoluble case of a donor area that is not supplied or devastated by important withdrawals that have left many gaps behind them during other interventions. It also makes it possible to solve other situations that cannot be resolved with commonly practiced techniques. We are talking about the restoration of large alopecias.

New hair will grow from the portions of follicular units transplanted and the portions of follicular units left in situ, after a nine-month cycle. The follicular units will reproduce and regenerate exactly the same as the original one. Once the donor area has been regenerated, if the patient wishes, a new operation can be repeated after a period of nine months.

HST EXCLUSIVELY IN HAIR SCIENCE CLINICS

It is in the Hair Science Clinics that HST is practiced exclusively, under the control of doctors trained in the Hair Science Academy of Maastricht where the research center where this miraculous method that led to the patent was born is based. The samples, we said, are grafts the size of 0.5 millimeters, the size of a needle puncture. The very small wounds that are created with extractions have a very fast healing process.

POST-OPERATIVE COURSE

After two or three days, the surface of the head is healed. No bleeding. The patient’s head will therefore not be bandaged at the end of the operation. The post-operative period does not present any particular difficulties. When the hair begins to grow after a few days, the patient will be presentable socially. Given the extremely small size of the grafts, it becomes possible to transplant with a high-density result in the donor area, placing the grafts very close to each other.

The grafts are sparse and devoid of tissue and would not survive after being extracted. Energy is then given to the grafts so that they can survive. They are immersed in a patented pharmaceutical composition that contains extracellular matrix. The permanence of the grafts in this medium is at least two hours after extraction.

THE REGENERATED DONOR AREA

The follicular units taken from the donor area are insensitive to dihydrotestosterone and are therefore not subject to fall. Once transplanted, these portions of the follicular unit will give birth to hair that will not fall out. The implant in the receiving area is made by creating very small housing holes. It is there that the withdrawals that have already been revitalized will be inserted.

The hair thus reconstituted will be natural-looking, without scars, and of high density. HST is also used to reconstruct eyebrows and beards. Scars such as those caused by FUT FUSS transplantation, also known as strips, can be restored using this method. HST is also used in reconstructive surgery to restore skin burns, as various scientific publications attest.

THE NECESSARY PRE-OPERATIVE CONSULTATION

The patient must undergo a preventive trichological examination. During this visit, the capillary restoration project is defined together with the patient. Often the patient’s expectations are very high and difficult to meet with commonly practiced techniques. The consultation will allow the patient to ascertain what his or her condition is, especially in relation to his or her donor area. This awareness will allow the patient to realize his real potential while, unfortunately, other unscrupulous operators allow him to consider unattainable dreams. The number of follicular units available is limited.

The promise of miraculous results is unethical, and often this promise is made more attractive by extremely low costs. The damage that patients too often suffer after surgeries performed in countries with poor hygiene regulations and by unqualified operators is difficult to repair. The patient is made aware of his real potential as his donor area allows. This observation must be put in relation to the area to be restored. The donor areas that have the highest potential because they have a significant capital of follicular units, can allow an extraction of 4 to 6 thousand follicular units. This donor area can no longer be exploited later, otherwise it will be super-exploited. In cases of alopecia vasta, Norwood 4 or higher, techniques that move follicular units from one part of the head to the other, will achieve partial or incomplete restoration results by disappointing the important expectations promised.
